What forms does a person ordered to surrender firearms, dangerous weapons, or a concealed pistol need to fill out for proof of compliance?

House Bill 1840 requires a person that is ordered to surrender firearms, dangerous weapons, and/or a concealed pistol license under RCW 9.41.804 must file proof of compliance with the order within five judicial days of the entry of the order, using the forms developed by the Administrative Office of the Courts. The following forms have been developed and/or updated by the Pattern Forms Committee regarding proof of surrender; they are located on the Washington State Court Forms site under the following menu options:
