Where can I find a sample (proof copy) of the uniform infraction citation forms?

Proof copies of the Notice of Infraction Form and the Criminal Citation Form are located on the Legal Resources/Notice of Infraction/Citation Forms page of the Inside Courts website.

When forms are revised, the Uniform Infraction/Citation Committee does allow the court to use up its existing stock when possible. However, due to legislatively mandated changes, or the extent of the changes, it is not always possible.
